NET Transportation
Through the NET (Non-emergency transportation) program the Logan County Department of Job and Family Services (LCDJFS) provides gas cards to help eligible Medicaid consumers get to their covered medical appointments.
To qualify for a NET gas card, you must have:
Gas cards also are available to persons who do not qualify for Medicaid but must transport a Medicaid-qualified child or other Medicaid-qualified family member to an appointment.
You must request a NET gas card before a medical appointment. Reimbursements will not be made after appointments. Reimbursements may be made, however, after an emergency room visit or labor and delivery.
Don’t have a driver’s license?
In that case, the person who will take you to your medical appointment must accompany you to your first NET appointment and present their driver’s license and proof of insurance to receive a gas card. Whenever you have a change of drivers while you’re in the program, you must notify the Social Services staff.
How to sign up
You may sign up for NET on a walk-in basis between 8 a.m. and 3 p.m. on Tuesdays and Wednesdays and between 8 a.m. and noon on Fridays. If coming the day of a medical appointment, arrive at LCDJFS at least an hour before a local appointment or at least two hours before an out-of-town appointment. NET appointments may be arranged at other times if determined by the agency to be an emergency.
The value of the gas cards varies based on location of the recipient’s medical appointment.
- An open Medicaid case
- A valid driver’s license
- Current auto insurance
- Proof of a qualified, upcoming appointment
